Final Report

Once grantees have completed their project, they must complete a final report.  Grantees have one month from the time the project is finished to complete and provide the following forms to the Neighborhood Connections program.  The forms are meant to be easy to complete.  If you have questions about them, or would like assistance in completing them, please contact Neighborhood Connections at (216) 393.4640 or e-mail us at Neighborhoodconnections@clevefdn.org.

Grant Agreement

Grantees must provide a signed copy of the following document to the Neighborhood Connections program in order to receive their grant check.  If you have a fiscal agent, they must sign the document as well.  As there are signatures on this form, you must provide the original signed copy to Neighborhood Connections (not by email).

This form may be mailed or delivered to the offices of Neighborhood Connections, 1422 Euclid, 15th floor, Cleveland 44115 (the Hanna Bldg.).

Revised Budget Form

All Neighborhood Connections grantees must also fill out this form, and return it to the program.  On this form is a place for you to provide us with updated contact information.   Also, we need to know when your project will be completed (within a year from the time grant money is first available).

For grantees either who did not receive the full amount that they requested, or have made changes in the project budget, you must also complete a revised budget page.  As always, if you need assistance in properly completing this form, please don't hesitate to contact us.
